What dermal fillers are
Over time, the skin on our faces loses resilience and starts to develop wrinkles and creases, and the underlying fat tissue can start to thin. Dermal fillers are gel-like substances that restore a younger look when injected below facial skin. Different types create different effects: some soften and smooth lines, others add volume to hollow areas or plump the lips.
Most filler formulas include substances that occur naturally in the body, including hyaluronic acid and poly-L-lactic acid, and all are FDA-approved for cosmetic use. We can also use your own fat tissue to create a long-lasting filler, harvesting fat cells with liposuction, processing them, and injecting them where you want more volume.

The most asked-for injectable we do, and the one where restraint matters most.
Lip filler adds volume, and it can define the vermilion border and the cupid’s bow. What it cannot do is change the distance between your nose and your lip. If that distance is what bothers you, a lip lift is the operation that addresses it, and filler will not substitute for it.
How much you need is a question of proportion to the rest of your face, not a fixed amount. A conservative first treatment is almost always the right starting point: it is straightforward to add at a second visit, and much harder to undo an overfilled lip that has stretched the tissue.
Most lip filler lasts somewhere between six months and a year, depending on the product and on you. Hyaluronic acid fillers are reversible, which is part of why they are the right choice here.
We use Restylane Kysse where softness and movement matter, Juvéderm Volbella for subtle volume and fine lip lines, and Restylane Silk for definition of the border. Which one suits you is decided at your appointment, not before it.
Where fillers are used

Each area has its own anatomy, its own product and its own limits. Naming what is bothering you is more useful than naming a filler, and the plan usually treats more than one area at a time.

A soft or receding lower face reads as heavier than it is. Jawline filler can sharpen the border in the right candidate; where the underlying projection is the problem, a chin implant addresses it properly and permanently.
Tear troughs
Hollowing beneath the eye casts a shadow that reads as tiredness regardless of how rested you are. This is the most technically demanding area on the face, product choice matters, and not everyone is a candidate.
Non-surgical rhinoplasty
Filler placed along the bridge or at the tip can straighten a profile, soften a dorsal hump, or lift a tip that has dropped — without surgery and without recovery. It suits specific corrections rather than a change in size, and the result is temporary. The nose is among the least forgiving areas to inject, with a blood supply that leaves little margin for error, and Dr. Sandel takes referrals for it from other injectors. Where the concern is breathing or the underlying structure, surgical rhinoplasty is the right answer instead.

Folds visible with the face completely still are volume problems rather than muscle problems, which is why a neuromodulator does very little for them. Filler placed above the fold often works better than filler placed in it.
