Chemical Peels (35% TCA Peels and Phenol Peels)
35% TCA (trichloroacetic acid) is the most common deep chemical peel performed at the Sandel Duggal Center. Dr. Sandel has years of experience performing these peels for patients who have extensive sun damage on the face, neck and chest. It has a good benefit for improving fine lines an wrinkles, sun spots and discolorations. Dr. Sandel uses this peel exclusively in the neck and chest and it has been shown that this one of the most effective single treatments for improving the skin in this area.
The depth of the treatment is determined by the concentration of the chemical. Many “dayspa” TCA peels are around 10% or less. This is good for superficial skin smoothing, but it doesn’t penetrate to the depth that is required for significant improvement in skin color or texture. 35% TCA is much more effective and should only be performed by a surgeon specializing in these procedures.
The TCA peel is most commonly performed in the operating room in combination with other cosmetic procedures. The skin is cleaned and prepped with Acetone to remove any natural oils that may be present. The 35% TCA is applied with a cotton applicator or gauze in a single or double pass. After a few minutes, the skin will develop a “frost” that signals the end of the peeling process and depth of penetration. Once this is achieved, the skin is then covered with a thin layer of Aquaphor to protect the skin while the peeling takes place over the course of 5-7 days.
Patients will then notice the skin peel over the following week. The skin will darken on the surface and then “peel” away to reveal healthy skin underneath. This peeling process is not painful. Aquaphor is used during this time to keep the skin protected and moist. The skin will appear a little pink and splotchy for a few more weeks… but this can be concealed with makeup if needed.
