What can I do to make my face look less tired and look more healthy and rejuvenated?
There are 3 main characteristics of the aging face:
- Loss of volume
- Loss of overall skin tone and elasticity
- Loss of structural support resulting in drooping, sagging or redundant skin
In order to restore a more youthful appearance, each of these three characteristics should be treated. There isn’t one single procedure that can address all three of these characteristics at the same time. Therefore, to restore youthfulness to the face, depending on your goals, we have to select which of these characteristics we are trying to address.
Restoring Lost Volume to the Face
Volume is one of the easiest things to address in our office, without surgery. Volume can be added to the midface, eyelids, cheeks and perioral areas in order to soften smile lines, elevate the cheek and restore contour to the jawline. The goal is to restore volume that is lost with aging thus creating a very natural and proportional result.
There are many types of injectable fillers that can be used and they are some of our most common treatments. When adding fillers to the correct places, patients typically get a “non-surgical” facelift result. Fillers are temporary, however many patient experience great results lasting about a year or longer, depending on the type and location of filler used.

For a longer lasting, more permanent surgical improvement, implants or fat injections can be used. The implants used in our center have proven to be very safe with minimal risks. They are easy to perform and the results are permanent. They include midfacial volume (cheek) implants, chin implants, prejowl implants and jawline or mandibular implants. They are used to augment the bone structure of the face, providing natural results. Soft tissue implants such as those used for lip enhancement, are not recommended and are not performed in our center.
If surgery is a good option for you, implants may be used at that time to restore facial volume in needed areas. The most common implant used is the prejowl implant used to improve results from facelifts. In certain patients, the chin looks like it has lost continuity and fallen off the front of the jawline. A depression develops in front of the jowl. Prejowl implants fill this area and gives facelift patients a subtle but much improve jawline “strength” that makes their results exceptional!
Chin implants are commonly used during rhinoplasty (nose-job) surgery. The improves the profile resulting in a more confident and lifted appearance.
Fat injections can be a nice alternative to implants in the midface region. Fat is harvested from the neck or abdomen and reinjected into the midface

Improve Overall Tone & Elasticity
The second characteristic of aging is the loss of skin elasticity. This is mostly caused by sun damage over time. The skin itself has an inherent ability to shrink wrap or contour to the underlying facial structure. As the skin loses this ability over time, fine lines and wrinkles start to form. This is mostly evident around the eyes, mouth and cheeks.
To restore the inherent elasticity and smoothness of the skin, one of the most powerful procedures that can be performed is a fractionated CO2 laser treatment. At The Sandel Duggal Center for Plastic Surgery in Annapolis, Dr. Sandel and Dr. Duggal use state-of-the-art CO2 lasers, such as the Lumenis Ultrapulse Encore CO2 laser, in order to rejuvenate the skin.
This can be a dramatic, one-time treatment that can last for years. CO2 lasers are highly effective in removing brown spots, precancerous lesions, fine lines and wrinkles and making the skin firmer and tighter as a whole. Lighter CO2 laser treatments are also used to maintain healthy skin in younger patients. There is no better treatment for consistent, natural results in skin rejuvenation!

Tighten & Lift
Tighten & Lift Sagging Skin
The third characteristic of aging involves drooping of the skin and facial features, which causes excess skin around the eyes, as well as across the jawline and underneath the neck.
Although lasers are wonderful at tightening the skin, if there is any redundancy or drooping of the skin, the best option is a surgical lift. This can be in the form of an eyelid lift, such as a blepharoplasty, or a lower face and neck lift, commonly referred to as a facelift.
This surgical procedure involves small incisions that are well placed and well hidden to allow the removal of the redundant skin, as well as recontouring and restoring the underlying facial structure, to give a very natural appearance.
Eyelid lifts and brow lifts give wonderful, permanent results than help patients look less tired and more refreshed. Regardless of the procedure that we choose; conservative, natural, and long lasting results are the key.
